Getting Started

Who should I call to get started?

If you’re interested in Midtown Oaks Post Acute, give our Admissions department a call. We will discuss our admissions process, answer any questions you have, and discuss your needs. We will also give you the opportunity to speak with some of our staff. 

Please contact us to discuss by phone or schedule a video chat. We’d love to get to know you better, answer any questions you might have, and have the opportunity to introduce you to a few members of our team!

What articles of clothing should we bring?

When moving into our facility, you should bring at least seven days’ worth of clothing and comfortable shoes, preferably ones with rubber soles. You should also bring pajamas and undergarments. You should ensure that you or your loved one’s name is clearly written in permanent ink on the items. We will log these in the patient’s inventory log.

Can I bring personal belongings?

Yes, we encourage you or your loved one to bring personal items like decorations, photos, and blankets to make the resident’s stay more comfortable. We don’t recommend bringing valuables like jewelry or cash.

Do I need to bring my medications?

In most cases, medications are coordinated through our clinical team and pharmacy partners. Our admissions staff will provide specific instructions prior to admission if any medications or documentation should be brought with you.

Insurance & Billing

What insurances does Midtown Oaks accept?

We accept private pay, all PPOs, and a wide range of insurances and HMOs, including Sutter Health, United Healthcare, Blue Cross Blue Shield, Medicare, and Medi-Cal. Our insurance experts will take time to help you understand your plan benefits.

Will Medicare cover my stay in the facility?

Residents may qualify for Medicare coverage if they meet Medicare eligibility requirements, including a qualifying hospital stay and physician order for skilled care. Coverage amounts and length of coverage vary. Our admissions team can help explain Medicare benefits and answer questions regarding eligibility.

Who do I talk to about questions in regard to financials, billing, and/or admission paperwork?

We employ a full-time Business Office Manager for all financial and billing concerns. Should you have any questions regarding these matters please make an appointment to see our Business Office Manager by contacting the front desk/receptionist.

The Admissions Coordinator will guide you through the necessary forms required on admission and review the rules and regulations regarding Medicare, Medicaid, and various insurances.

How do I pay my bill?

We offer online bill pay. You can make a payment via our portal.

Does Midtown Oaks offer rehabilitation?

Yes. Midtown Oaks Post Acute offers rehabilitation to help residents gain strength and confidence to successfully return home or go to another facility. All services are customized based on the resident’s needs and abilities. We offer the following rehabilitation services:

When will the doctor see me?

After admission, doctors will typically see residents within 24-48 hours. One of our skilled nurses will do an evaluation and then present any medical concerns with your doctor.

How often will I have therapy?

Depending on their condition and when they are admitted, most patients will start rehab within 24 hours of admission and will receive therapy up to 5 times per week.

How long is a typical stay?

Each person’s needs are different, and rehabilitation and recovery times will vary. Patient progress is reviewed weekly by our Interdisciplinary Team (IDT) of nurses, therapists, and dietitians. Our main goal is to help you reach your highest level of independence.

Amenities

What is your dining program like?

At Midtown Oaks, we’re proud to offer restaurant-style dining. Diets are designed by our full-time, in-house Registered Dietitian.

Can my loved one receive mail?

Yes. Mail is delivered directly to residents each weekday. Family members are also welcome to take advantage of the Send a Greeting feature on our website to share messages and encouragement with loved ones.

Do your offer activities?

We offer daily activities for a wide variety of interests. For your convenience, there is an activity calendar posted in your room. Please contact the Activities Director for additional information.

Are private rooms available?

Yes, we offer both semi-private and private rooms that include bathrooms for your privacy and comfort. Our recovery suites also include personal climate control, in-room WiFi, televisions, privacy screens, safety features, and a nurse call speaker for direct communication.

Policies

Is smoking permitted in the facility?

Midtown Oaks Post Acute is a non-smoking community. Designated smoking areas are available outdoors for those who choose to smoke.

What is your visitor policy?

At Midtown Oaks, we believe that family and friends play a critical role in our residents’ emotional health and physical wellbeing. Visiting hours are typically 8 am to 8 pm. Unfortunately, we cannot accommodate overnight visitors.

Are pets allowed to visit?

Yes, pets that are up to date on vaccines are allowed to stay with you. You will need to provide a copy of the animal’s vet records to our staff.

Can I bring in my loved one’s favorite food?

Possibly. Because some residents follow specialized diets, we ask families to consult with the nursing team before bringing food. Any food kept in resident rooms should be stored in properly sealed containers.

Questions & Concerns

What role does Social Services play?

Our social services team supports residents and families throughout their stay. Services include care planning, discharge planning, coordination of outside resources, emotional support, family communication, and assistance with services such as dental, vision, podiatry, audiology, dementia assessments, psychological services, and therapy referrals.

What do I do if I have concerns about my loved one’s care?

Our primary concern is that the residents feel comfortable here and that their needs are met. You or your loved one should feel secure and comfortable with our staff, particularly those providing direct care. If you have any concerns or praise for our staff, please feel free to contact our Social Services department. Our staff will be able to address any concerns you may have regarding any staff members. You can also ask to speak with the Director of Nursing and/or Administrator.

How do you protect patients, staff, and visitors from COVID-19 and other infectious diseases?

Our team is highly trained and experienced in caring for high-acuity patients and infectious diseases like COVID-19. Our dedicated infection prevention nurse helps ensure we follow current infection prevention guidelines and best practices. This includes educating all staff about the appropriate use of personal protective equipment (PPE) such as N95 masks, surgical masks, gloves, and gowns and when to use COVID-19 protocols required by local department of public health and/or the CDC.
