Was really ticked off because I had a few items of nine stolen. I don’t own anything pricey Why did my injection medicine ,Kesiempta shot come up missing locked in the refrigerator with a price tag of $10, 000. As well as some scrunchies and other personal items as well as my Driver’s license?¿ Who does that?

I am so grateful for this facility and all of their staff! My dad was moved here less than a week ago after a hospital stay. He was in a previous facility that was awful and he was not getting proper care. I am beyond impressed with their wound care nurses and nursing team in general. My dad has a severe pressure wound and since getting to midtown oaks it looks leaps and bounds better! Everyone is so nice, so kind and has been great about communication and discussing my dad’s care. This is a night and day experience from his previous facility. I don’t think words can convey how grateful I am for this place and the care my dad is receiving .

My dad was worsened at this facility. He was admitted in January 2026 from a seizure and remained till beginning of March 2026. Because we could not go every day due to 3-4 hours distance, we had to call. Communications was terrible especially with the social worker. We asked the social worker to provide us with hospice options in our town. It took a long time to get a document signed after my dad was released. We saw the physical therapist force my dad to sit up seeing fully well that he was in pain. When my dad was sent home, we did not have much support. The social worker just requested a nurse to visit instead of requesting hospice which anyone could clearly see he needed hospice instead. It was the nurse who immediately saw that my dad needed hospice and he told me which hospice to call. Something the social worker along with the facility nurses, physical therapist and doctors could have done in January 2026 when my dad was first admitted at their facility. After my dad was released from their facility, the social worker would not answer my messages and phone. After 2 weeks, I reached out directly to her manager to request the appropriate doctor document to be sent to the local hospice to help my dad. Such a waste of time waiting to get her to provide something simple while my dad was suffering more and more. My dad passed away end of March 2026, and I really feel this facility made him worse.

The facility is clean, well maintained and provides a comfortable environment for residents. The therapy team does an excellent job helping residents regain strength and independence. The communication between staff and families is outstanding.

Friendly staff, a convenient location and thus far my 85 year old mother has not complained/has no complaints. However, I am alarmed with the overwhelming feces smell in the facilities upon entering. Why?

This was our family’s first experience with a skilled nursing facility. My father is not a willing patient at all and it was a difficult necessity to have him recover and regain his strength before returning home after a broken hip. MidTown Oaks’ wonderful staff not only helped my father recover physically, they instilled confidence in him and made a tough situation a whole lot easier. I am thankful to the entire team from the housekeepers, CNAs, nurses, dietary staff, therapy team, and care management team for the thorough and thoughtful care provided in my dad’s recovery journey!

Everyone has been so delight here, my CNA’s we all very attentive and helpful at all times. I felt well rounded care from everyone. Food is above average. I enjoyed all the activities , I would recommend this place to everyone, and would come back if I ever need rehab again.

I like the auto check in. Makes it smoother when you’ve already been there before. I had to ask front desk a room number, the two staff members were busy doing something so I had to flag them to let them know I had a question. I was only standing there for about 10 seconds before saying excuse me though so once I got their attention I did feel noticed/heard/acknowledged. Always a pleasant experience at Midtown Oaks-best SNF I’ve ever visited.

I’m beyond disappointed with this facility, they did not contact me and im the power or attorney for a patient, let alone tell me that they had a in date from medical insurance. They told me the night before she was to be discharged. She is unable to walk. She cannot stand she can’t do anything and she needed to stay there longer, but they sent her home and to the point she can not care for her self at all I do not recommend this place and the case manager of social services LIZ has not and will not return any of my calls. I will be reporting this and more to the state board and county this is ridiculous and poor management and they refused to listen to their own employees that work in physical therapy when they told her (LIZ) she wasn’t ready to go home and she can’t walk or stand BUT NO SHE REFUSED TO LISTEN!!
